About the Role
The Master’s Social Worker (MSW) in Hospice provides patient- and family-centered emotional, social, practical, and financial support during end-of-life care.
Key attributes: compassionate, resourceful, culturally aware, and dedicated to providing dignified, holistic end-of-life support.
Responsibilities
- Assess psychosocial needs of patients and families
- Offer counseling and crisis intervention
- Educate on advance directives
- Connect families with community resources
- Collaborate with the interdisciplinary hospice team
- Develop and update personalized care plans
- Ensure accurate and timely documentation
Requirements
- Master’s degree in Social Work (MSW)
- Hospice or end-of-life care experience
- Valid driver’s license and reliable transportation for home visits
