Social Worker MSW - Case Manager

About the role

This position promotes patient and family wellness, improved care outcomes, access to appropriate hospital and community resources, and manages, supports, and develops comprehensive transitional care plans for patients with complex and psychosocial needs in Inpatient and Emergency Department (ED) environments. The role focuses on promoting quality of care and effective transitions planning through collaboration with all service team members, physicians, patients, external agencies, and families.

Responsibilities

• Promote patient and family wellness.

• Manage, support, and develop comprehensive transitional care plans for patients with complex and psychosocial needs.
• Collaborate with service team members, physicians, patients, external agencies, and families to ensure quality care and effective transitions planning.

Requirements

• Master's Degree in Social Work from an accredited social work program (Required).
• 1 year of experience in an acute or post-acute setting (Required).

Qualifications

• LMSW - Licensed Masters Social Work (Preferred).
• Accredited Case Manager (ACM) (Preferred).
• CCM - Certified Case Manager (Preferred).
