Social Worker MSW

We are seeking a qualified Social Worker MSW to provide comprehensive, patient-centered care management in an acute care setting. Key responsibilities include assessing patient needs, coordinating care, discharge planning, documenting interventions, ensuring regulatory compliance, and advocating for patients.
