About the role
Join our team as a Case Management Acute Care Social Worker (MSW) at Hillcrest Medical Center, a 656-bed hospital that includes The Alexander Burn Center, The Peggy V. Helmerich Women’s Health Center, Kaiser Rehabilitation Center, advanced cancer treatment technology, and the Oklahoma Heart Institute.
The medical social worker integrates clinical social work within the case management system, serving three main purposes:
- Act as the patient advocate, liaising between patient and medical personnel.
- Address emergent needs of patients and families through counseling, education, and connecting them with community services and resources to facilitate long-term health, self-responsibility, and self-sufficiency.
- Provide clinical social work services, case management, and discharge planning to ensure efficient and effective use of hospital services while maintaining compassionate, quality care.
The social worker will assist in creating a comprehensive bio-psycho-social patient profile, use relevant psychotherapeutic interventions, and develop and implement treatment plans tailored to the patient’s specific needs. They are responsible for ensuring discharge planning is appropriate, timely, and communicated to the patient, family, and healthcare team.
Requirements
- MSW from a program accredited by the Council on Social Work Education (CSWE).
- Active member of the National Association of Social Workers (preferred).
