Social Worker MSW

About the Role

Provides comprehensive social work services to PACE participants as a key member of the PACE Interdisciplinary Care Team.

Responsibilities

• Presents social work concerns, intervention suggestions, and treatment plan recommendations to the interdisciplinary team.
• Confers with participants and families regarding expectations, long- and short-term goals; coordinates and attends family meetings and nursing home care planning meetings.
• Reviews significant psychosocial, behavioral, and financial events during morning meetings and recommends care plan adjustments.
• Intervenes in crises, addressing emergency needs related to illness, disability, or loss of independence; advocates for participants' rights and self-determination.
• Develops and revises participants’ plans of care as part of the Interdisciplinary Team, integrating the Social Work treatment plan.
• Assesses participants, families, and home environments for social work needs; documents care and progress in the Electronic Medical Record (EMR).
• Assists in identifying needs related to social, psychological, financial supports, counseling, and housing options; collaborates on applications for public assistance and housing placement.
• Performs pre-enrollment, initial, semiannual, and annual assessments; develops individualized care plans and attends intake, assessment, and clinical case conferences.
• Maintains communication with team members and nursing homes to coordinate client care.
• Assists in developing and implementing social work policies and procedures; participates in QAPI programs and task forces.
• Educates team members on community resources, Medicaid, psychosocial issues, and applicable regulations.
• Counsels paraprofessional staff on psychosocial and communication issues; supervises BSW and MSW students as appropriate.
• Adheres to compliance policies and promotes workplace compliance.
• Ensures high-quality care by following professional standards and PACE/D&TC Emergency Preparedness Plan requirements.

Qualifications

• Licensed by the NYS Education Department as a Social Worker.
• Master of Social Work degree from an accredited program.
• Minimum of one year of professional experience in geriatric care, long-term health care, or community services for aging populations.
• Excellent written and verbal communication skills.
• Strong organizational, problem-solving, and conflict resolution skills.
• Proficient in MS Office programs and able to learn new software rapidly.

An equivalent combination of education and experience may substitute for the above requirements.

Pay

$63,000 - $78,000 per year.
