About the role

The Social Worker (MSW) at Del Sol Medical Center provides non‑clinical, biopsychosocial assessments and interventions for inpatient floors and the outpatient emergency department. The role includes crisis intervention, high‑risk screening, emotional support, referrals for financial assistance, and post‑acute care placement for patients with behavioral health or resource needs. The position serves intensive care units, Trauma, Oncology, NICU, Transplant, and the Emergency Department, responding to physician and discharge‑planning consults throughout the facility.

Responsibilities

• Collaborate with RN case managers, acute‑care nurses, physicians, insurance companies, and other staff to identify and address patient needs.
• Identify and remove barriers to discharge and recovery, including financial, emotional, psychosocial, medical, and spiritual factors.
• Maintain thorough knowledge of Medicare, Medicaid, and private‑payer regulations and processes.
• Understand managed‑care concepts and length‑of‑stay management to support fiscal responsibility.
• Perform comprehensive assessments and develop discharge plans for high‑risk patients with psychosocial or discharge‑planning needs.
• Coordinate and facilitate meetings between the care team and patients, families, and caregivers.
• Assist patients and families in making difficult decisions while honoring their preferences and values.
• Provide psychosocial intervention, assistance, and support for patients/families coping with chronic, acute, or terminal illness.
• Engage in consultation, discharge planning, education, counseling, advocacy, and information referrals to address emotional, social, and financial aspects of care.
• Offer emotional support during complex or emotionally charged situations such as end‑of‑life discussions, terminal illness notifications, death notifications (with physician), and crisis interventions.

Qualifications

• Master’s degree in Social Work (LCSW) or related licensure (LMSW/RCSW‑I, Licensed Master Social Worker/Registered Clinical Social Worker‑Intern).
• No travel required.
• No prior experience required; entry‑level candidates are welcome.
