Responsibilities

  • Serve as a critical subject matter expert, bridging the gap between direct clinical care and command-level health strategies.
  • Conduct brief, targeted biopsychosocial assessments.
  • Deliver evidence-based interventions in 30-minute appointments using a stepped-care model.
  • Treat chronic conditions, insomnia, acute stress, and health behaviors.
  • Triage/refer complex cases to Specialty Mental Health.
  • Complete concise electronic medical record documentation within 72 hours of the visit.
  • Provide immediate, succinct feedback to Primary Care Managers (PCMs).
  • Actively drive same-day "warm handoffs" and increase clinic-wide service penetration.
  • Review daily PCM schedules ("scrubbing lists") and participate in morning huddles.
  • Offer continuity consultation to monitor long-term patients and prevent relapse.
  • Develop and lead educational classes focused on coping skills and behavior change.
  • Train primary care clinic staff to recognize symptoms and optimize PCMHI referral pathways.

Qualifications

  • U.S. Citizenship required.
  • Master of Social Work (MSW) from a CSWE-accredited institution.
  • Active, unrestricted Clinical Social Work License (e.g., LCSW, LICSW) permitting independent practice from any U.S. jurisdiction.
