Social Worker- Medical - Home Health Care - 24 Hours/Week - Days

About the role

Under general supervision, this position implements treatment programs to assist homebound clients and their families to adjust to personal and environmental difficulties, which predispose illness or interfere with obtaining maximum benefits from medical care.

Responsibilities

• Provides individual and group counseling to clients and their families
• Assists clients and their families in accessing community resources
• Solves problems related to client care and provides solutions
• Analyzes and develops policies and procedures related to client care

Requirements

• Master's degree in social work required
• One (1) year of social work experience in a health care setting required
• Licensure in the State of Michigan as a Social Worker (LMSW) required
• Current State of Michigan driver's license required

• Knowledge of the principles of human behavior
• Individual and group counseling techniques
• Relevant statutes and regulations
• High levels of social sensitivity, personal emotional adjustment, and verbal skills
• Professional level of analytical skills
