Responsibilities

• Provide high-quality, compassionate care to home health patients
• Work under the direction of the Director of Clinical Services and Clinical Team Manager
• Deliver assigned functions of care in a professional and patient-focused manner
• Utilize knowledge of community, state, and federal resources to support patients
• Make a meaningful impact in patients' lives every day

Requirements

• Master's degree in Social Work (MSW) or Licensed as a Clinical Social Worker (LCSW) from an accredited school required for consideration
• 1-2 years of experience required, with strong community knowledge
• 1+ year of home care experience preferred
• Ability to exercise initiative, sound judgement, and independent decision-making
• Strong collaboration and communication skills
