Social Worker LMSW / Hospice - PRN

About the Role

Licensed Social Worker in hospice care, serving a designated territory including Bay, Saginaw, Arenac, Midland Counties, and surrounding areas.

Responsibilities

• Conduct compassionate initial psychosocial, spiritual, and pre-bereavement assessments for patients, families, and caregivers.
• Provide ongoing emotional support, counseling, and grief assistance throughout the care journey.
• Identify and connect families with community resources, including financial aid.
• Guide families through advanced directives with sensitivity and clarity.
• Assist with facility placement when appropriate.
• Maintain accurate and timely clinical documentation at patient residences and agency records.
• Participate in interdisciplinary team and IDG meetings to develop holistic care plans.
• Manage caseloads while ensuring compliance with relevant laws, regulations, and policies.
• Contribute to on-call duties during evenings and weekends as needed.

Requirements

• Master’s degree from an accredited school of social work.
• State licensure and/or certification as required.
• Minimum of 2+ years in healthcare or social services, with hospice or palliative care experience preferred.
• Comfort with grief, loss, and end-of-life discussions.
• Strong knowledge of community resources and care coordination.
• Excellent documentation and compliance skills.
• Self-motivated with minimal supervision.
• Valid driver’s license and willingness to travel locally for patient visits.
