Social Worker LMSW / Hospice

About the Role

A licensed social worker providing hospice care to patients and families in specified regions, focusing on psychosocial support, resource coordination, and compassionate end-of-life guidance.

Responsibilities

• Conduct compassionate psychosocial, spiritual, and pre-bereavement assessments.
• Provide ongoing emotional support and counseling to patients and families.
• Identify and connect families with community resources, including financial assistance.
• Guide families through advance directives with sensitivity and clarity.
• Assist with facility placements when appropriate.
• Maintain accurate clinical documentation and contribute to interdisciplinary care planning.
• Participate in on-call duties and ensure compliance with relevant laws and regulations.

Requirements

• Master’s degree in Social Work from an accredited institution.
• State licensure/certification as required.
• At least 2 years of social service experience in healthcare or social services.
• Experience in hospice or palliative care preferred.
• Strong communication skills, especially around grief and end-of-life topics.
• Knowledge of community resources and care coordination.
• Excellent documentation skills and attention to compliance.
• Self-motivated with ability to work independently.
• Valid driver’s license and willingness to travel locally.
