Social Worker - LMSW

Role Overview: The heart transplant social worker provides specialized psychosocial care to patients with advanced heart failure, mechanical circulatory support, and heart transplantation. Key Responsibilities Supports patients and caregivers from evaluation through post-transplant follow-up.Addresses clinical, emotional, logistical, and financial needs, including insurance and disability resources.Performs urgent evaluations, manages long-term device and transplant readiness, and collaborates within a multidisciplinary team. Qualifications & Skills MSW degree from an accredited program and 2 years of hospital social work experience.Active LMSW license in Arizona required.Preferred experience in transplant social work and insurance-driven care pathways.Strong clinical judgment, organizational, communication, and problem-solving skills.Ability to work efficiently under pressure and collaborate across disciplines. Additional Requirements: Certification in Transplant Social Work (preferred), or willingness to pursue; experience with chronic illness psychosocial patterns; ability to navigate complex cases and prioritize effectively.
