Job Summary and Responsibilities

You will plan, assess, and intervene for inpatients, outpatients, and emergency department patients. Perform comprehensive assessments and develop integrated treatment plans that address medical, social, and resource issues impacting patients, families, and at-risk populations. Manage multiple priorities, engage in creative, compassionate, and ethical problem-solving, and collaborate with healthcare teams to develop and implement patient-centered care plans. Provide crisis and patient/family interventions, high-risk screening, brief counseling, and referral for financial or other resource needs. Facilitate family/patient meetings, assist with post-acute placements, and engage appropriate agencies or community resources for high-risk patients. Establish and maintain professional relationships with various stakeholders.

Requirements

  • Master's Degree from a School of Social Work accredited by the Council of Social Work Education and maintain current California Clinical Social Worker License (LCSW).
  • At least two years of clinical and/or medical social work experience.
