About the role

As a Licensed Social Worker (LSW), you will perform vital clinical assessments and design individualized treatment plans tailored to help patients achieve their personal goals. You will join a multidisciplinary team that delivers meaningful, life-changing care to individuals suffering from anxiety, panic, depression, suicidal ideation, schizophrenia, and other complex mental health challenges.

Responsibilities

• Provide direct clinical social work services to patients and their families in an interdisciplinary team approach.
• Complete initial psycho-social assessments on assigned patients.
• Provide psychotherapeutic counseling with individuals, families, or groups as needed.
• Give clinical consultations as needed and provide patient-centered advocacy.
• Collaborate with interdisciplinary team and manage complex discharge plans.
• Build and maintain relationships with internal/external resources.
• Make referrals to community agencies and entitlement programs.
• Continue ongoing education.
• Ensure continuity of care for patients with complex post-acute needs and maximize patients' and families' abilities to meet continuing needs within their social and financial framework.
• Assess each patient’s age-specific needs to provide individual treatment and care, understanding all stages of growth and development.
• Report identified safety issues such as hazardous environments (e.g., damaged floors/walls/ceiling tiles/unsecured areas) and medical errors.

Requirements

• Graduation from an accredited graduate school of social work with an MSW (Master’s in Social Work) degree.
• State licensure required by the NJ State Board of Social Work Examiners.
• Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) depending on years of social work experience and clinical coursework (LSW may render clinical social work services under the direct supervision of an LCSW).
• Three or more years of experience in a hospital setting preferred.
• Successful completion of all Orientation Programs.

Pay

The anticipated annual salary range for this position is $76,500.00 - $94,300.00 per year. The compensation offered will depend on several factors, including the candidate's educational background, skills, and professional experience.

Schedule

Full-time, day shift.
