About the Role

This class of positions encompasses professional, supervisory, or administrative work of varying degrees of difficulty and responsibility in operating a social work department. The role ensures patient-centered quality care and services to support better outcomes and improved patient experience, in accordance with all applicable local, state, and federal regulations and social work standards of care.

Responsibilities

  • Screens charts for historical and clinical information, conducts psychosocial evaluations through interviews with patients and/or identified surrogates, and formulates appropriate social work treatment plans.
  • Provides individual and group counseling, as well as concrete social services, in healthcare or community settings within NYC.
  • Consults with professionals in other disciplines to determine and implement a multidisciplinary approach to patient care, develop treatment plans, evaluate patient progress, and create discharge and aftercare plans.
  • Provides outreach services, makes referrals to governmental, public, or community agencies, and prepares patients to accept services. Offers direct social work services on high-risk cases and may escort patients for community activities and referrals.
  • As a mandated reporter, documents and reports suspected cases of child abuse or neglect to the New York State Central Registry.
  • Appears and testifies at administrative and court hearings, as necessary.
  • Assists in discharge planning and evaluates alternate care arrangements for patients.
  • Participates in staff conferences, social work meetings, special studies, committees, research projects, and internal/external quality assurance/performance improvement (QA/PI) activities.
  • Prepares reports, keeps appropriate records, and ensures compliance with NYC Health + Hospitals policies, national standards, and regulatory requirements.
  • Mentors less experienced social work staff and serves as a resource in advanced social work practice.
  • May supervise graduate and undergraduate social work students, volunteers, and other auxiliary personnel.
  • May provide clinical services under the supervision of a New York State LCSW, licensed psychologist, or psychiatrist, or independently as an LCSW.

Requirements

  • Valid license as an LMSW or LCSW issued by the NYSED.
