About the role
Woodhull Medical and Mental Health Center is committed to addressing healthcare issues pertinent to North Brooklyn residents. The center focuses on preventive care and promoting healthy lifestyles through various accessible locations.
Duties & Responsibilities
- Screens charts for historical and clinical information, conducts psychosocial evaluations through interviews with patients and/or identified surrogates, and formulates appropriate social work treatment plans.
- Provides individual and group counseling, concrete social services, and makes referrals to other governmental, public, and community agencies within health care settings or community settings within NYC.
- Confers and consults with professionals in other disciplines to develop multidisciplinary approaches to patient care, evaluate patients' progress, and develop discharge and aftercare plans.
- Outreaches to patients, prepares them for services, and provides direct social work services on high-risk cases, including escorting patients for community activities and referrals.
- Documents and reports suspected cases of child abuse or neglect to the New York State Central Registry.
- Attends and testifies at administrative and court hearings as necessary.
- Aids in planning institutional or hospital discharge and other post-discharge care, evaluates alternative care arrangements for patients, and assists in developing discharge plans.
- Participates in staff conferences, special studies, committees, and research projects, and engages in internal/external QA/PI activities, programs, and trainings.
- Prepares reports and maintains appropriate records to ensure ongoing compliance with NYC Health + Hospitals policies and procedures, national standards, and external regulatory requirements.
- Mentors and assists less experienced social work staff in social work techniques and serves as a resource in various aspects of advanced social work practice.
- May supervise graduate and undergraduate social work students, volunteers, and other auxiliary personnel, as needed.
- Assists in the provision of clinical services under supervision of a New York State LCSW, licensed psychologist, or psychiatrist, or performs such functions independently as a LCSW.
Minimum Qualifications
- Valid license as an LMSW issued by the NYSED; or valid license as an LCSW issued by the NYSED for Assignment to Level II.

- Equipment/Machines Operated: Telephone, Desktop computer.
- Computer Programs/Software: EPIC, Microsoft Office Suite, Internet applications and assigned portals.
