• Provide comprehensive psycho-social assessment, intervention, management, and support
• Assess for discharge planning and aftercare needs of the at-risk patient population and their families
• Address social determinants of health
• Work collaboratively with the multidisciplinary team

Qualifications

• M.S.W. from a Social Work program accredited by the Council on Social Work Education (CSWE)
• M.S.W., L.M.S.W., L.C.S.W. or eligible
• 3 years relevant post-master's experience, preferably in a hospital and/or healthcare environment
• New M.S.W. graduates who are L.M.S.W. eligible will be considered for appointment to entry-level (SL2) positions at a commensurate salary after obtaining a Limited Permit
• Incumbent must obtain L.M.S.W. within 12 months of receipt of the limited permit

Preferred Qualifications

• Previous knowledge and experience with the psychiatric population
• Experience with suicide interventions
• Knowledge of OMH regulations

Schedule

Monday-Friday, days, with occasional on-call/holiday coverage on rotation, based on operational needs.
