Social Worker Inpatient - Case Management
SUMMARY OF POSITION
Functions as part of an interdisciplinary team to maintain continuity of patient care for identified patient populations from admission to discharge, utilizes appropriate criteria for inpatient care and plans timely discharges. Provides emotional support to patients and their support systems in managing complex medical conditions in an inpatient setting. Responsible for crisis intervention, individual, family, group, bereavement, and end of life counseling. Actively participates in designing and implementing institutional programs/initiatives to improve patient access to resources, quality of care, cost reduction efforts and patient satisfaction.
MINIMUM QUALIFICATION(S)
- Masters of Social Work degree required.
