About the Role

We are seeking a compassionate and dedicated Social Worker III to provide comprehensive psychosocial support and care coordination for children, adolescents, and their families. In this role, you will conduct assessments, develop individualized treatment plans, facilitate therapeutic and educational groups, provide crisis intervention, and collaborate with multidisciplinary teams to promote recovery and positive patient outcomes. The ideal candidate is highly organized, patient-centered, and committed to maintaining the highest standards of safety, documentation, and professional care while supporting patients through complex life challenges and transitions.

Responsibilities

  • Conduct psychosocial assessments of patients and develop effective treatment plans that apply therapeutic principles while ensuring patient safety.
  • Obtain information on patient needs, skills, interests, and limitations through observation and interviews to develop and implement treatment plans.
  • Provide crisis intervention and assist families in understanding the implications and complexities of life situations and their impact on lifestyle.
  • Participate in multidisciplinary treatment team meetings to discuss psychosocial assessments, treatment planning, and ongoing discharge planning.
  • Identify, assess, and coordinate patient cases, anticipating needs related to illness, hospitalization, finances, education, and home/community services.
  • Facilitate education, process groups, support groups, and referrals as requested.
  • Conduct intakes and assist in coordinating placements within facility programs.
  • Develop and execute clinically relevant activities in coordination with activity professionals.
  • Promote recovery, symptom reduction, and the achievement of the highest level of social functioning.
  • Communicate with or aid family members in understanding and supporting the patient.
  • Manage a caseload and assume responsibility for accurate and timely patient chart documentation to comply with facility and accreditation standards.
  • Supervise social work interns completing field placements, as required.
  • Provide direction and guidance to social services staff or other staff members, as required.
  • Demonstrate a positive, empathetic, and professional attitude toward customers at all times; acknowledge and work to resolve complaints when patient needs are not met.
  • Recognize that patient safety is a top priority.
  • Perform other functions and tasks as assigned.

Requirements

  • Master's degree in Social Work from an accredited college or university.
  • Three or more years of related experience working with a comparable patient population (children and adolescents).
  • State licensure as required by the state in which the facility operates, which may include: Licensed Master Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), or Licensed Social Worker (LSW).
  • CPR and de-escalation/restraint certification (training available upon hire and offered by the facility).
  • First aid certification may be required based on state or facility requirements.
