Social Worker II MSW
Responsible for providing therapy, case management services, and linkages to community agencies to meet patients' social and emotional needs. Identifies and assesses client patient cases and anticipates their needs related to illness, hospitalization, finances, education, and home or community services. Serves as a liaison between health care facilities, community resources, and government agencies.
About the Role
Provides individual and group therapy to children and adolescent patients in an outpatient community psychiatry treatment clinic located within an academic medical center. Interviews patients, families, and other providers to assess the patient's level of functioning, strengths, deficits, and emotional status. Develops a comprehensive picture of the patient’s treatment needs and serves as a member of the treatment team in diagnostic evaluation and identification of treatment objectives.
Responsibilities
- Provide evidence-based treatment for patients at the Children's Day Treatment (CDT) program.
- Deliver group, family-based, and individual therapy.
- Provide therapeutic support and develop treatment plans, monitor progress, and document outcomes to meet compliance and billing requirements.
- Facilitate discharge planning to successfully return the patient to community resources.
- Work within a multidisciplinary team of therapists, medical providers, and families to ensure comprehensive treatment.
- Participate in weekly clinical team meetings with a multi-disciplinary team of medical providers and therapists.
Requirements
- Master's degree in a social work or social welfare program accredited by the Council on Social Work Education.
- MSW licensure by the South Carolina Board of Social Work Examiners.
- Certification in case management preferred.
Pay
Pay grade: Health-27 (salaried position).
Schedule
Scheduled weekly hours: 40.
