Position Overview The Care Manager plays a vital role within the interdisciplinary team, responsible for assessing, planning, advocating, and coordinating patient care from admission to discharge. They ensure safe transitions post-hospitalization, focusing on patient safety, experience, and quality of care. Utilizing performance metrics and best practices, the Care Manager oversees transition planning and length of stay. Qualifications & Skills Education: Master's in Social Work or equivalent, with licensing in the state of employment (LMSW, LCSW).Experience: 0-2 years in an acute care hospital setting preferred.Certifications: BLS (CPR) required at hire or within 90 days; case management certification preferred. Responsibilities Assess and coordinate patient care from admission to discharge.Advocate for patients and ensure safe, efficient transitions post-hospitalization.Maintain high standards of safety, quality, and patient experience through performance metrics.
