Social Worker II - Acute Geriatric (FT)

Position Summary: Provides psychosocial support and counseling services to patients and families, ensuring emotional and social needs are met. Collaborates with the interdisciplinary team to develop and implement care and discharge plans, advocating for patient rights and utilizing community resources effectively. Key Responsibilities Conducts comprehensive psychosocial assessments and updates care plans accordingly.Serves as advocate for patients with mental health or substance abuse issues, coordinating care with community providers.Provides therapy, patient education, and discharge planning tailored to behavioral health populations.Ensures safety and confidentiality, adheres to infection control, patient rights, and safety regulations.Communicates progress and goals with patients, families, and team members, supporting adaptation and treatment compliance.Maintains accurate documentation, participates in quality improvement, and stays current with regulatory standards. Qualifications & Skills Master's in Social Work (MSW) or equivalent, with current state licensure (LMSW, LCSW).0-2 years of experience in a healthcare setting preferred.Strong assessment, communication, critical thinking, and computer skills.Ability to work with diverse age groups from neonatal to geriatric.Effective interpersonal skills and ability to interact with various stakeholders.
