About the Role

The Social Worker I for the Multipurpose Senior Services Program (MSSP) provides case management activities to frail, older adult clients. Working as part of a team with a Registered Nurse, this role focuses on care navigation and case management support, conducting in-depth intakes and assessments to address psychological, social, physical, and environmental concerns. The goal is to help Medi-Cal-eligible older adults remain safely in their homes by identifying limitations and activating necessary resources. Services are primarily field-based, delivered within the client's home setting.

Responsibilities

• Respond to intake callers to gather initial information and provide referrals.
• Review monthly service delivery invoices, verify client receipt of services, and process invoices for payment.
• Provide outreach and support to clients in collaboration with the Registered Nurse.
• Assist clients in applying for basic grants and other government benefits.
• Coordinate family or other informal support for assigned clients.
• Arrange transportation for doctor’s appointments or other client needs.
• Process monthly food vouchers for assigned clients.
• Provide timely documentation in compliance with funding requirements and electronic health records.
• Participate in multi-disciplinary team meetings and weekly individual supervisory meetings.
• Perform other related duties as assigned.

Requirements

• Minimum of a Bachelor’s degree in social work, psychology, counseling, rehabilitation, gerontology, sociology, or a related field.
• Two years of experience working with frail older adults.
• Current unrestricted California Driver’s License and proof of auto insurance.
• Must pass a criminal background check (Live Scan fingerprinting) prior to employment.

Qualifications

• Care management and field-based service delivery experience preferred.
• Ability to target and navigate resources for the aging population preferred.
• Bilingual in Russian, Spanish, or Farsi is preferred.
• Experience conducting in-home visits is strongly preferred.
• Excellent verbal and written communication skills, including ability to use telehealth platforms.
• Computer literate in MS Office applications.
• Culturally sensitive and able to work with a multicultural client population in a non-judgmental manner.
• Able to work as part of a team and maintain professional boundaries.
