Social Worker I

About the Role

This is a full-time position for a Social Worker. The role involves demonstrating professional responsibility, complying with personnel and hospital safety policies, and maintaining confidentiality in all interactions with patients, families, personnel, and the public.

Responsibilities

• Gather timely assessment information, including components of home environment, support systems, coping skills/stressors, strengths, grief/loss issues, adjustment to illness, identified needs, and goals.
• Develop and carry out intervention plans and make appropriate referrals.
• Document patient assessment and intervention data using Social Services documents and Continuum of Care forms in the medical record, covering assessment, goals, interventions, options provided to patient/family, and referrals/outcome.
• Demonstrate working knowledge of community resources, financial assistance programs, Advance Directive laws and processes, guardianship/Mental Health commitment processes, and Medicaid/Medicare programs.
• Take initiative in gathering resource information.

Qualifications

• Licensed as a Social Worker or Independent Social Worker through the Iowa Department of Public Health and the State of Illinois (preferred).
• Master’s degree from a graduate school of social work accredited by the Council on Social Work Education, with specialization in clinical practice (preferred).
• Bachelor’s Degree in Social Work or related field (required).
• One to two years of social work experience in a healthcare-related setting (preferred).
• Excellent communication, diplomacy, and people skills (required).
• Acute observation skills (necessary).
