Social Worker I

About the role

Responsible for AltaMed Managed Care participants assigned to the team. This position works closely with providers and nurses to coordinate services and advocate for participants.

Responsibilities

• daily notes
• conducting family conferences
• assisting with community referrals
• regular contacts with participants
• incident reporting

Requirements

• A bachelor’s or master’s degree in social work from an accredited university is required.
• Minimum of 2 years of experience in case management and social advocacy required.
• Experience working with the homeless population is preferred.
• Managed Care Experience preferred.
• Bilingual English/Spanish strongly preferred.

Pay

Compensation $26.78 - $32.48 hourly. Compensation Disclaimer Actual salary offers are considered by various factors, including budget, experience, skills, education, licensure and certifications, and other business considerations. The range is subject to change.
