Social Worker Hospice & Palliative Care
About the role
The Hospice and Palliative Social Worker provides psychosocial support and counseling for patients and families facing life‑limiting illness. The social worker coordinates psychosocial and community resource needs in collaboration with the Hospice and Palliative Care Interdisciplinary Group (IDG). The hospice and palliative Social Worker demonstrates critical thinking, analysis, and clinical judgment in all aspects of hospice and palliative care delivery for patients and families experiencing life‑limiting illness. This includes addressing the social, emotional, financial, and spiritual needs of patients and families to improve quality of life and reduce suffering. Like our patients, the Main Line Health Family encompasses a wide range of backgrounds and abilities.
