Qualifications

• Master of Social Work (MSW) required
• Current State Licensure in Social Work
• Minimum of one year’s experience in health care or hospice setting
• Valid driver’s license and proof of insurance is required
• Bi-lingual (English/Spanish) preferred

Responsibilities

• Performs the psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensures that identified needs are addressed in the patient’s plan of care
• Provides an assessment in the patient’s identified residence and assistance when this is not safe, and another plan is required
• Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers; assist team members as needed to ensure that this level of service is consistently provided
• Counsels patient and family/caregivers and staff as needed in relationship to stress and other identified coping difficulties
• Provides accurate and timely documentation of patient services to reflect the plan of care
• Provides assistance with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver; obtains resources from community social, health, pharmaceutical, and welfare agencies to assist in meeting patient/family needs
• Serves as liaison between patients and families/caregivers and community agencies

Pay

Salary range: $62,000–$80,000 (offer based on experience)

Schedule

Monday–Friday, 8:30 AM–5:00 PM (after-hours and on-call requirements)
