About the role
The Home Health Social Worker provides psychosocial support and counseling for patients receiving home health services. The social worker coordinates psychosocial and community resource needs in collaboration with the interdisciplinary team of nurses, therapists, dietitians, and home health aides. The social worker demonstrates critical thinking, analysis, and clinical judgment in all aspects of care delivery for patients and families experiencing acute and chronic illness. This includes addressing the social, emotional, financial, and spiritual needs of patients and families in order to improve quality of life and home safety.
Responsibilities
- Provide psychosocial support and counseling for patients receiving home health services.
- Coordinate psychosocial and community resource needs in collaboration with the interdisciplinary team.
- Demonstrate critical thinking, analysis, and clinical judgment in all aspects of care delivery.
- Address the social, emotional, financial, and spiritual needs of patients and families.
- Improve quality of life and home safety for patients and families.
Requirements
- One year social work experience in health care setting preferred. Home Health or other community experience preferred.
- Master's Degree from an accredited School of Social Work.
- CURRENT LICENSED SOCIAL WORKER IN THE STATE OF PENNSYLVANIA REQUIRED.
- Valid driver's license and automobile insurance.
- CURRENT CPR CERTIFICATION.
Qualifications
- Must have a Master's Degree from an accredited School of Social Work.
- Must be licensed as a Social Worker in the State of Pennsylvania.
- Must have current CPR certification.
Skills

- Strong organizational and time management skills.
- Knowledge of community resources and services.

Pay
$71,427.20 - $110,656.00

Full Time Day Shift; 8:30 am to 5:00 pm; Monday through Friday
