Social Worker - Healthcare for Homeless Veterans Outreach

Summary

The HCHV Outreach Social Worker functions independently in a community-based setting, managing and delivering assistance for chronically mentally ill Veterans in crisis. They are expected to have a broad and comprehensive knowledge of community resources and familiarity with local, state, and federal benefits.

About the Role

Responsible for locating and engaging homeless Veterans through Community Integration Services, completing psychosocial assessments, developing plans of care, and collaborating with internal and external service providers to address the Veteran's needs.

Responsibilities

• Engage homeless Veterans through visits to encampments, shelters, and other unfit living conditions.
• Provide information about Community Integration Services and transport Veterans to/from VAMHS if needed.
• Complete psychosocial assessments under supervision and develop a plan of care.
• Contribute to the development of the plan of care and set achievable treatment goals.
• Collaborate with internal and external service providers to address Veteran needs.
• Educate Veterans and family members/significant others about available services and assist them in establishing appropriate service referrals.
• Develop knowledge of community-based homeless resources and competency in accessing these services.
• Expand collaborations with community-based providers to provide access to other social services.
• Assist in addressing treatment barriers for the homeless Veteran population.
• Be the subject matter expert on VA and community-based homeless resources.
• Refer homeless Veterans for appropriate levels of care.

Requirements

• Master's degree in social work from a CSWE-accredited school.
• Licensure or certification as a social worker at the independent practice level.
• Experience in a clinical setting.
• Knowledge of community resources and ability to make appropriate referrals.
• Ability to conduct psychosocial assessments and develop treatment plans.
• Ability to establish and maintain effective working relationships.
• Knowledge of medical and mental health diagnoses, disabilities, and treatment procedures.

• Ability to implement treatment modalities in individual, family, and group settings.
• Ability to communicate effectively with clients, staff, and community agencies.

• Knowledge of medical and mental health diagnoses and treatment procedures.
• Ability to provide consultation services to new social workers and staff.

Qualifications

• Basic requirements include a master's degree in social work and licensure/certification as a social worker.
• May qualify based on being covered by the Grandfathering Provision for current VHA employees.
• Must demonstrate knowledge, skills, and abilities as outlined in the KSAs.
