We are seeking a Social Worker for a travel assignment in Long Beach, California.
Responsibilities
- Provide psychosocial assessments and identify barriers impacting care transitions.
- Support patient and family meetings related to care planning and transitions.
- Document assessments, plans, and communications in the medical record.
- Ensure continuous support and documentation throughout the assignment.
Schedule
40 hours weekly with 10-hour shifts.
Requirements
- Minimum of 2+ years of social work experience.
