About the role

Promotes optimal social functioning of the participants through direct and indirect services.

Serves as a resource to participants enrolled in the Prisma Healthy Start program, helping them manage social and emotional issues that affect their healthcare treatment.

Supervises and works alongside Resource Mothers (Paraprofessionals) to develop, organize, and evaluate the Social Work program.

Works as part of an outreach team to engage with the community and recruit potential participants for the Prisma Healthy Start Program.

Responsibilities

  • Supervises paraprofessionals and develops Service Plans/Treatment Plans to ensure participants are receiving proper care and services.
  • Recruits and admits potential participants from the community to participate in the Prisma Healthy Start Program.
  • Reviews all visitation forms and documentation generated by paraprofessionals to ensure that services are being provided and meet Medicaid requirements for billing.
  • Audits and monitors quality assurance activities, including chart audits and supervisory visits.
  • Provides crisis management with the paraprofessionals on a regular basis to monitor progress towards goals, problem-solve, and implement strategies to promote maternal and child health.
  • Directs case management with the paraprofessionals on a regular basis to monitor progress towards goals, problem-solve, and implement strategies to promote maternal and child health.
  • Collaborates with members of the multi-disciplinary team to facilitate effective and efficient use of community and financial resources utilized by the participants served by the Prisma Healthy Start program.
  • Assesses and follows up on psycho-social and financial needs of participants based on individualized service plans and potential needs of the participants and their family unit.
  • Performs evaluations of clinical data as part of an interdisciplinary team, interpreting data and making recommendations based on results.
  • Educates the community about prenatal and child healthcare, as well as parenting skills, through meetings with formal and informal community groups.
  • Performs other duties as assigned by supervisor.

Requirements

  • Master’s degree in Social Work from an accredited school required.
  • Three (3) years of experience in a Social Work-related area. Medical healthcare experience preferred.
  • Must be licensed LMSW or Licensed Practitioner of the Healing Arts (LPHA).
  • Must possess Heart Saver Knowledge.
