Social Worker FT, Day

About the Role

Promotes optimal social functioning of participants through direct and indirect services. Ensures that while participants are enrolled in the Prisma Healthy Start program, clients receive help in dealing with social and emotional problems which interfere with their ability to respond to healthcare treatment. Supervises and collaborates with Resource Mothers (Paraprofessionals) to assist in the development, organization, and evaluation of the Social Work program. Works as part of an outreach team.

Responsibilities

• Supervises paraprofessionals and develops Service Plans/Treatment Plans to ensure participants receive proper care and services.
• Recruits and admits potential participants from the community to participate in the Prisma Healthy Start Program.
• Reviews all visitation forms and documentation generated by paraprofessionals to ensure services meet Medicaid requirements for billing.
• Audits and monitors quality assurance activities, including chart audits and supervisory visits.
• Provides crisis management with paraprofessionals to monitor progress toward goals, problem-solve, and implement strategies to promote maternal and child health.
• Directs case management with paraprofessionals to monitor progress toward goals, problem-solve, and implement strategies to promote maternal and child health.
• Coordinates care in collaboration with members of the multi-disciplinary team to facilitate effective and efficient use of community and financial resources.
• Provides assessment and follow-up of psycho-social and financial needs of participants based on individualized service plans.
• Works within an interdisciplinary team to evaluate clinical data, interpret results, and make recommendations.
• Educates the community regarding prenatal healthcare, child healthcare, and parenting skills through meetings with formal and informal community groups.
• Performs other duties as assigned by supervisor.

Requirements

• Master’s degree in Social Work from an accredited school.
• Three (3) years of experience in a Social Work-related area; medical healthcare experience preferred.
• Must be licensed as an LMSW or Licensed Practitioner of the Healing Arts (LPHA).
• Heart saver certification.
