Social Worker for Nephology, Kidney Transplant and Dialysis

About the role

At Cincinnati Children’s, the Social Service Division employs over 150 licensed clinical social workers who provide ongoing support and counseling, crisis intervention, advocacy, education, and connection to community resources for patients and families. Guided by core values of self‑determination, dignity, respect for all beliefs and cultures, and strengthening patient and family strengths, the division serves patients throughout inpatient hospitalizations and outpatient settings. Social workers assist families coping with medical and/or psychiatric conditions by addressing emotional, social, and financial well‑being, navigating the complex medical system, and providing comprehensive psychosocial support.

Responsibilities

• Psychosocial Assessment- Performs psychosocial assessments taking into consideration family, social, and medical history. Maintains current knowledge of medical and mental health diagnosis. Report suspicions of abuse and neglect to legally mandated authorities for investigations. Completes documentation with accuracy and clarity.
