Purpose of the Role
Social Worker must have excellent clinical, oral and written communication skills; must provide casework, outreach, referral and advocacy services to families. Functions as part of a medical and rehabilitation team to plan for the patients' needs. Written intake, case summary and statistical reports are required.
Responsibilities
- Understands and promotes Elizabeth Seton Children’s Mission, Vision, and Core Values to ensure application with organizational policies and procedures.
- Participate in intake evaluation and assists families with follow-up of initial treatment plan.
- Provide ongoing casework services, therapeutic intervention and work as part of clinical team.
- Coordinate the services of CRC, Inc. with the families and appropriate involved agencies.
- Assist with communication between family members/caregivers and medical staff during clinic appointments.
- Provide information to clinic families and make appropriate referrals to community support services.
- Carry small caseload of Children’s Waiver cases in partnership with leading Health Home under supervision of SW Manager.
- Support Social Work Manager in all activities pertaining to departmental functions, including Transportation.
- Maintain departmental policies, procedures, objectives, quality assurance, safety, environmental and infection control.
- Comply with established CRC, Inc. policies and procedures regarding quality assurance, safety, environmental and infection control, confidentiality of patient information, customer service, attendance, punctuality, dress code, professional conduct, annual health assessments, etc.
- Participate in all departmental in-services and staff development activities.
- Perform other duties as assigned.
Qualifications
- MSW from an accredited University in Social Work.
- Licensed Master of Social Work required.
- Minimum 1 year of experience in a pediatric clinical setting preferred.
- Bilingual Spanish speaking strongly preferred.
Employee Requirements
- Must meet or exceed all general health standards and requirements for the position as established by employer in compliance with NYSDOH and other regulatory agencies if appropriate.
- Must be able to stand and move freely while treating and supervising patients.
- Must be able to drive and travel throughout Westchester and Putnam Counties for Care at Home Program.
- Must be able to assist in the evacuation of patients as necessary.
- Must conduct self in a professional and ethical manner, and maintain good staff relationships as part of an interdisciplinary team.
- Must be able to lift/carry a minimum of 35 pounds.
Working Conditions
- Office space; another room is available for testing if desired.
- Meetings take place in the office, conference room, or classrooms.
- A percentage of the work (for Health Home Program) takes place in the patients’ homes or other locations within Westchester County.
Pay
$60,000/Yr. to $70,000/Yr.
