Job Expectations
Aid in planning, organizing, implementing, evaluating, and directing the operations of the Social Service Department. Provide medically related social work services to help residents achieve or maintain optimal physical, mental, and psychosocial health.
Job Responsibilities
- Intervene with residents, families, and physicians
- Offer psychosocial services
- Administer vision, hearing, and dental services
- Conduct assessments and develop care plans
- Coordinate discharge planning
Requirements
Prioritize 2 years of experience in a Long Term Care or Hospital setting.
Qualifications
- Obtained a 4-year degree
- Held a Social Worker license in the State of Texas
Knowledge / Experience
Minimum of 2 years experience working in a Skilled Nursing Facility or Hospital setting.
Licensure / Certifications
Hold a Social Worker license.
