Green Oaks Nursing & Rehab is seeking an experienced and dedicated Licensed Social Worker.
About the role
Assist in planning, organizing, implementing, evaluating and directing the overall operation of the Social Service Department. Provide medically related social work services so that each resident may attain or maintain the highest practicable level of physical, mental and psychosocial well-being.
Responsibilities
- Resident/Family/Physician Intervention
- Psychosocial Services
- Vision, Hearing, Dental Services
- Assessments & Care Planning
- Discharge Planning
Requirements
- 4-year degree
- Licensed Social Worker in the State of Texas
- At least 2 years experience in a Skilled Nursing Facility or Hospital (preferred)
