Social Worker CSW LSW

Responsibilities

• Affords assistance to residents with emotional adjustment to the facility.
• Interviews new residents and documents social history and assessments as directed.
• Completes the social services portion of the MDS, care planning, baseline, and other relevant tasks.
• Refers residents to psychological services as necessary.
• Schedules and attends interdisciplinary care plan meetings, morning meetings, utilization reviews, and other meetings.
• Writes summaries and progress notes as required.
• Conducts pre-discharge evaluations, arranging for visiting nursing services, community resources, and other discharges.
• Follows up with residents post-discharge.
• Writes discharge summaries as assigned.
• Develops a professional relationship with each resident and their families.
• Educates residents and representatives about advanced directives.
• Adheres to all Social Services Regulations.

Qualifications

• Minimum Master's Degree.
• Must obtain a CSW and/or LSW license.
• One to three years of professional experience and/or training, or equivalent combination of education and experience.
• Experience with dialysis patients is preferred.
