About the Role
The Social Worker is a member of the interdisciplinary team collaborating with the Physician. They utilize the care process consistent with their certification to initiate interventions within their scope of practice, providing concrete, non-clinical services. The role involves overseeing the coordination of the interdisciplinary plan of care and ensuring its consistency with the medical treatment plan.
Responsibilities
- Assessment: Identify, gather, and organize subjective and objective data about the patient, family, or significant others to make referrals and implement a social care and action plan.
- Counseling: Apply social work methods and values to advise and guide individuals, families, or groups to enhance, protect, or restore coping capacity within their social environment.
- Consultation: Engage in discussions and evaluations focusing on data, goals, and interventions with families, agencies, businesses, organizations, groups, and communities for problem-solving.
- Planning: Develop a documented plan of care by identifying actual or potential problems and desired outcomes (goals). The plan directs patient/family and interdisciplinary team actions, including:
- Identifying and prioritizing problems related to biophysical, psychological, environmental, self-care, educational, and social status.
- Setting goals consistent with identified problems to guide patient care and health promotion.
- Providing education aligned with identified problems and goals to empower patients.
- Administer all patient care activities according to standards, procedural guidelines, and organizational policy.
- Support performance improvement initiatives and provide leadership to other members of the patient care team.
- Participate in educational activities to ensure continued professional and personal growth.
Requirements
- Education: Bachelor's degree from an accredited school of social work.
- License/Certification: CSW (Certified Social Worker) certification required. Valid driver's license required for roles in Case Management, Mission Health Services, and Adult and Child Partial Programs.
- Experience: Minimum of 1 year of experience, preferably within healthcare.
- Proficiency in service applications preferred at hire; training will be provided, and proficiency will be required within 30-60 days of hire.

- Provide care appropriate to the age of patients, as defined on the unit-specific Assessment and Evaluation Tool.
Work Environment
- Potential exposure to hazards and risks of a hospital environment, including infectious diseases and hazardous substances.
- Requires considerable walking and standing.
- Frequent periods of driving a motor vehicle.
- Essential functions are listed on the department-specific Assessment and Evaluation Tool.
This position reports to department leadership.
