Social Worker, Complex Grief and Bereavement in the Hospice & Palliative Care Program

About the role

The role involves providing complex grief and bereavement support to patients and their families within the hospice and palliative care program. The ideal candidate should have extensive experience in social work and a compassionate nature.

Responsibilities

• Provide individual and group counseling sessions to patients and their families dealing with complex grief and bereavement issues.
• Collaborate with interdisciplinary teams to develop comprehensive care plans for patients.
• Offer emotional support and guidance to patients and their families during difficult times.
• Document all interactions and progress notes in accordance with professional standards.

Requirements

• Bachelor’s degree in Social Work or related field.
• At least 5 years of relevant experience in social work.
• Valid driver’s license and access to reliable transportation.
• Proficient in Microsoft Office Suite.

Qualifications

• Excellent interpersonal and communication skills.
• Strong organizational and time management abilities.
• Ability to maintain confidentiality and handle sensitive information.
• Experience working in a healthcare setting preferred.

Pay

$X per hour, depending on experience.

Schedule

Full-time, Monday through Friday, with occasional evening and weekend shifts as needed.
