About the Role

Crescent Hospice is seeking a patient-centered Licensed Master Social Worker (LMSW) to join our team. In this role, you will deliver exceptional social work services to hospice patients and their families, providing counseling and guidance to support emotional well-being and coping capacity during challenging times. A $5,000 Sign On Bonus is available.

Schedule & Coverage Area

• Schedule: Monday – Friday, 8:00 AM – 4:30 PM
• Coverage Area: Florence and Sumter

Responsibilities

• Assess and communicate psychosocial status to the RN Case Manager and interdisciplinary group
• Evaluate medical needs, home situations, financial resources, and community support
• Implement personalized social work plans and collaborate with the Bereavement Coordinator
• Shape care plans, attend interdisciplinary meetings, and address psychosocial stresses
• Provide empathetic counseling to patients and families

Qualifications

• Education: Master of Social Work from an accredited school of social work
• License: Licensed as a Social Worker from the State Board of Social Work; current unencumbered license in South Carolina
• Driver's License: Current Driver's License, vehicle insurance, and access to a dependable vehicle
• Experience Required: Minimum one (1) year experience as a social worker in a healthcare setting (long-term care or medical surgical/acute care setting preferred)
• Experience Preferred: Minimum one (1) year experience as a social worker in a hospice setting

Pay

$31.00 – $33.00 per hour (average pay range). The final compensation rate will be discussed and confirmed at the conclusion of the interview process, based on relevant experience and specific responsibilities.
