We are seeking a compassionate and dedicated professional to join our healthcare team. This position plays an important role in supporting the physical, psychological, and social needs of our residents while helping them make healthy and meaningful adjustments to the skilled nursing environment.
Responsibilities
- Build and maintain positive relationships with residents, staff, family members, responsible parties, and outside agencies.
- Generate enthusiasm, cooperation, and engagement among residents and members of the care team.
- Respond to Social Services inquiries and collect prior stay and general resident information as needed.
- Evaluate residents based on their backgrounds, interests, abilities, physical limitations, and individual needs to develop meaningful social service programs.
- Interview residents, families, friends, and responsible parties to gather relevant information.
- Review transfer forms, medical history, and physician documentation for accurate and appropriate information.
- Initiate discharge planning upon admission and provide updates at required intervals.
- Complete discharge planning and discharge plans of care when indicated.
- Complete initial social and emotional assessments and conduct quarterly reassessments or assessments following significant changes.
- Attend resident care plan conferences and actively participate in developing individualized plans of care.
- Create, review, and update care plans and progress notes.
- Work with residents, families, and interdisciplinary team members to establish goals of stay, plans to achieve those goals, and appropriate discharge plans.
- Implement social service interventions that address resident needs and support treatment goals.
- Connect residents with social supports, physical care resources, and appropriate community services to enhance quality of life.
- Support a clean, safe, dignified, happy, and healthy environment for residents.
Requirements
- Bachelor's degree required.
- Current LSW licensure required.
- LCSW or LICSW strongly preferred.
- Minimum of two years of experience working in a skilled nursing facility.
- Short-term rehabilitation and discharge planning experience required.
- Experience with behavioral management and/or geriatric populations preferred.
- Strong communication, documentation, assessment, and interpersonal skills.
- Ability to work effectively with residents, families, staff, and interdisciplinary team members.
