Duties/Responsibilities
- Manage a caseload comprised of medically and culturally diverse inpatient and outpatient families.
- Absorb and complete comprehensive biopsychosocial assessments, identify appropriate resources and referrals, provide supportive counseling, assist with discharge and transition planning, and serve as a liaison and advocate between the patient and various systems.
- Communicate with the interdisciplinary team and provide valuable input relative to intervention and assistance.
- Document information in the medical record consistent with departmental standards.
- Maintain a working knowledge of a variety of referral sources and community resources appropriate for the patient population within the state of Texas.
- Absorb and provide support during onboarding of new employees and ongoing support.
Requirements
- Master's degree in social work from an accredited (CSWE) Social Work Program required.
- Demonstrate Care Rite Values.
- Minimum 3 years of experience as a social worker and has the ability to perform job responsibilities independently.
- Texas State Board of Social Work Examiners (TSBSWE) licensure (LMSW or LCSW).
- Experience with developmentally delayed or neurologically impaired children is beneficial.
- Proven performance record that displays zero disciplinary actions within 12 months.

- Frequent use of telephone, fax, copy machine and computer.
