*Social Worker

Department: Social Worker • Facility: Resurrection Medical Center • Location: 7435 W Talcott Ave, Chicago, IL 60631

Responsibilities

• Provide emotional support, crisis intervention, and counseling to patients and families dealing with problems related to hospitalization, illness, disability, or trauma.
• Offer information, referral, and referrals for financial assistance and supportive services.
• Provide case management and milieu management.
• Complete psychosocial assessments.
• Assist in the development and re-evaluation of Treatment Plans.
• Work as a member of a multidisciplinary team and coordinate discharge planning.
• Complete necessary documentation and maintain clinical records for each client during their length of stay.

Requirements

• Bachelor’s degree in Social Work, Marriage and Family Therapy, or a related field.
• Current registration as an intern with the Board, as appropriate to education and licensing requirements.
• Minimum one (1) year of Inpatient or Outpatient Behavioral Health experience (preferred).
• Current BCLS certificate upon hire and maintain current (preferred).
• Master’s in Psychology, Social Work, Marriage and Family Therapy, or as a Licensed Professional Clinical Counselor (preferred).

Pay

A reasonable compensation estimate for this role is $28.42 to $41.17 per hour. The exact starting compensation will be determined based on factors such as skillset, years of applicable experience, education, credentials, and licensure.
