Job Summary

Department: Social Service
This job is for 8-hour shifts and five days each week, with the shift schedule being from 08:00 AM to 16:30 PM, a total of 40 hours per week.

Qualifications

• MSW
• Discharge Planning/Crisis Intervention/Monitor Length of stay for Goals
• Birth & Women's Unit/ED/Medical/Surgical/ICU Rotating throughout facility

Responsibilities

• Implement and monitor patient care plans.
• Maintain accurate records of patient conditions and communicate as necessary.
• Serve as a primary coordinator of all disciplines for well-coordinated patient care.
• Note and execute physician and nursing orders.
• Assess and coordinate patient's discharge planning needs with members of the healthcare team.
