About the role

Innovative Hematology, Inc. (IHI) offers comprehensive services and holistic care for patients with rare blood disorders and their families. The Social Worker plays a crucial role in protecting and improving the social and mental well-being of patients and families.

Responsibilities

  • Assess psychosocial needs of patients and families during comprehensive clinic visits.
  • Collaborate with multidisciplinary team to establish and implement a plan of care.
  • Provide psychosocial services during patient office visits or emergencies.
  • Educate patients and families about available community resources and coordinate services.
  • Contribute to discharge planning for hospitalized patients.
  • Engage in IHI events, Partners Program, camps, and other initiatives as requested.
  • Participate in national and regional committees to support organizational initiatives.
  • Maintain awareness of legislative changes affecting individuals with chronic disorders.
  • Communicate with external agencies and organizations to promote IHI services.
  • Coordinate with billing and insurance departments to resolve related issues and navigate Medicaid, Medicare, and disability systems.
  • Participate in continuing education and professional development activities.

Requirements

  • Master's degree in social work required.
  • Minimum 3 years of experience in complex case management in a healthcare setting.
  • Minimum 2 years of direct clinical experience providing therapy.
  • Valid Licensed Clinical Social Worker license in Indiana preferred, but Licensed Social Worker considered if currently pursuing clinical license.
  • Valid driver's license and auto insurance required due to occasional travel.
