Social Worker

About the role
We have an exciting full-time career opportunity for a caring, outgoing, organized, and detail-oriented Social Worker at Edenbrook Lakeside, a welcoming skilled nursing facility. In this role, you will support nursing home residents by promoting their overall well-being and addressing emotional and psychosocial needs. This includes conducting interviews and assessments, developing resident-centered care plans, and collaborating with staff to implement interventions that enhance quality of life.
Responsibilities
- Provide Social Services oversight, completing duties as assigned for the residents of Eden Senior Care.
- Work closely with the Marketing Director to coordinate the referral-to-admission process.
- Communicate with insurance companies to obtain prior authorization for admissions; keep a record of insurance updates and communicate with departments to ensure timely updates.
- Assist new residents and family members with the completion of Admission Paperwork.
- Provide information to residents/families regarding Medicare, Medicaid, and other financial assistance programs; assist with completion of Medicaid applications as necessary.
- Ensure PASRR is completed to state requirements.
- Participate in resident assessment—provide background information and social history as identified in the MDS process, in accordance with the RAI manual.
- Involve the resident and family members in planning objectives and goals; develop a written plan of care to meet identified goals.
- Attend and lead Behavior Meetings and monitor Gradual Dose Reductions (GDR) and consent for psychotropic medications according to state regulations.
- Complete timely progress notes on each resident.
- Maintain an open-door policy for residents, family members, and staff to address problems or needs; provide counseling or referrals as needed.
- Report concerns/grievances to the Executive Director and initiate investigations.
- Serve as an advocate for residents and their rights, helping them communicate needs, wants, and problems to appropriate departments and agencies.
- Participate in Monthly QAPI Committee meetings.
- Attend daily Stand-Up meetings; report pay changes, care conferences, and concerns to the IDT.
- Initiate and lead the discharge planning process; coordinate with outside resources such as DME, Home Health, and Hospice to ensure safe transitions.
- Issue denial letters according to state regulations.
- Assist in providing in-service education and orientation for employees regarding Resident Rights and social service needs.
- Perform all other duties assigned by the Administrator.
Requirements
- Bachelor’s degree in Social Work or a Human Services field (e.g., Social Services, Sociology, Special Education, Rehabilitation Counseling, Psychology).
- Current and valid Licensed Social Worker (LSW) certificate in the state of Wisconsin.
- At least one year of experience working as a Social Worker in a Skilled Nursing Facility (preferred).
- One year of supervised social work experience in a health care setting, working directly with individuals (preferred but not required).
- Able to promote cooperation between social services and other department employees.
- Professional demeanor over the phone and in person when representing the company.
- Excellent communication skills and pride in performance.
- Ability to write and give clear, concise instructions and remain calm during emergency situations.
