About the role
This is part-time position (24 hrs/wk) - split with 2 locations; Fremont (16hrs) and Grand Haven (8hrs)
Responsibilities
- Support and advocate for patients undergoing dialysis
- Help patients understand their rights and responsibilities
- Guide patients in managing the physical, mental, emotional and financial demands of End Stage Renal Disease
Requirements
- Master's degree in Social Work (MSW) required with a specialization in clinical practice
- Moves all state required regulations to practice in the dialysis setting
- Demonstrated knowledge of government and private insurance programs
- Basic computer skills in MS Word, Excel, PowerPoint and Outlook as well as functional proficiency with DaVita specific applications within 60 days
