About the role
Community Healthcare of Texas provides hospice and palliative care services, offering compassionate care for individuals living with serious illnesses while supporting their loved ones. As a Social Worker, you will provide therapeutic and emotional support, education, and resource information to patients and their families, while advocating for patients’ needs and wishes.
Responsibilities
- Assess the psychosocial needs of the patient and family members.
- Provide therapeutic counseling, emotional support, and education on end-of-life issues to patients and family members.
- Contribute to and execute the Plan of Care with the family and patient.
- Record daily documentation of visits and contact in accordance with policy.
- Provide referrals to community resources.
- Perform in-office and non-clinical administrative duties.
- Perform other duties as assigned.
Requirements
- Bachelor of Social Work with 1 year of applicable experience required; Master's degree preferred.
- Texas State license required.
- Reliable transportation and a valid driver's license.
- Hospice or healthcare experience preferred.
- Proficient computer skills required.
