Social Worker

About the role

The social worker will work with residents in the nursing home by identifying their psychosocial, mental and emotional needs along with providing, developing, and/or aiding in the access of services to meet those needs.

Responsibilities

• Working collaboratively with interdisciplinary team members
• Completing BIMS and Mood Assessments
• Documenting in medical chart
• Knowledge of Sigma Care Electronic Medical Record system
• Ability to work well with Residents and families
• Timely MDS and care plan completion
• Coinciding and implementing safe discharges
• Ability to problem solve

Requirements

• LSW, LCSW, or LICSW in the state of New York
• 1-year experience in SNF (preferred)
• Prior work with elders in a community or long-term care setting is preferred

Qualifications

• Strong skills in communication, assessment and social work methods and techniques
• A working understanding of social systems along with the ability to implement pertinent state and federal regulations

Skills
