What You'll Make an Impact In
Reduce Readmissions & Ensure Safety: Oversee complex discharge plans, ensuring patients have crucial resources at home, such as housing, follow-up care, and medications.
Provide Emotional Support: Deliver vital mental health counseling, offer crisis intervention, and connect patients to behavioral health services.
Provide Advocacy & Resource Expertise: Serve as a strong advocate for patients and families, coordinating various community resources to improve the patient's overall well-being post-discharge.
Minimum Requirements
- Licensure / Certification / Registration: Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM also accepted.
Certified Social Worker preferred. - Education: Bachelor's degree in Social Work required.
- Work Experience: 3 years of experience preferred.
Additional Preferences
- Masters Degree in Social Work;
- LCSW or LMSW;
- Acute Case Management preferred;
- Supervision available.
